Administration and Dosage:Unless otherwise prescribed by a doctor, the usual adult dose of Cozaar 50 mg is 50 mg given intravenously every 12 to 24 hours. The dose may be increased to a maximum recommended dose of 100 mg every 8 hours in patients with hepatic impairment. In patients with cirrhosis, renal function may be severely impaired in a patient taking cozaar with severe liver disease. If renal function is impaired, the dose may be increased to a maximum recommended dose of 200 mg daily in patients with mild to moderate liver disease, in patients with moderate to severe liver disease. In patients with severe liver disease, the dose may be increased to a maximum recommended dose of 400 mg daily in patients with moderate to severe liver disease, in patients with moderate to severe cirrhosis. In patients with severe liver impairment, the dose may be increased to a maximum recommended dose of 800 mg daily in patients with moderate to severe liver impairment, in patients with moderate to severe liver impairment, or, in patients with mild to moderate liver impairment, the dose may be increased to a maximum recommended dose of 2 mg/kg daily in patients with moderate to severe hepatic impairment, in patients with mild to moderate renal impairment, and the dose may be increased to a maximum recommended dose of 3 mg/kg daily in patients with mild to moderate renal impairment.
Administration:For patients requiring a dose above 120 mg/24 hours the maximum daily dose should be titrated up to 300 mg/day to achieve optimal therapeutic response. The daily dose may be increased to 300 mg to reach the maximum dose for patients with mild to moderate liver impairment.
Contraindications:Hypersensitivity to the active substance or to any of the excipients.
Dosage Contraindications:Patients who are receiving cozaar 50 mg, or those who are taking cozaar 50 mg, or those who have experienced any of the following, as may be listed here, require a dose above 120 mg/24 hours. If a dose above 120 mg/24 hours the maximum daily dose of cozaar 50 mg is necessary to achieve therapeutic response, the dose may be increased to a maximum recommended dose of 120 mg/24 hours.
Special Precautions:
Laboratory Tests Required:Blood samples should be separated into two portions. One blood sample is taken after a meal and should be taken at the same time each day. The second blood sample is taken on an empty stomach or after a meal and should be taken at the same time each day. No special measures are recommended for the blood test. The blood test should not be done if the patient has a known allergy to Cozaar 50 mg, or to any of the excipients listed above.
